What is Ketamine?
Ketamine is a class III scheduled controlled substance best known for its use in surgery as a general anesthetic. Ketamine is also used in various other areas of medicine, such as in patients with pain, anxiety, PTSD, treatment-resistant depression, suicidal ideations, bipolar disorder, and more.
Ketamine is commercially available only in very limited forms. Ketamine can be compounded into various dosage forms customized to the concentration or dose you need only by a qualified compounding pharmacy.

Precautions
Ketamine may impair mental and/or physical ability. Use care when driving or operating machinery. Do not take it with alcohol, sleep aids, tranquilizers, or other central nervous system depressants.
If you miss your regular dose of ketamine, take it when you remember but do not take a double dose to make up for the missed one. Do not exceed or change the prescribed dose.
Consult your healthcare practitioner if any prescription or over-the-counter medications are currently (or will be) used.
Contact your practitioner immediately if you experience serious side effects within 24 hours after you take ketamine.

Sterile & Non-Sterile Ketamine Forms
Ketamine Nasal Spray
Compounded ketamine nasal spray is a well-known dosage form and contains a different isomer than the commercial ketamine nasal spray, therefore has different advantages. Compounded ketamine does not require office administration like the commercial product.
Rapid onset: 5-10 mins
Ketamine Capsules
A good option for those sensitive to the taste of ketamine in all other dosage forms. The absorption and onset are slower than troches.
Variable onset: 30 mins-hours
Ketamine Rapid Dissolve Tablets (RDTs)
These dissolve faster than larger troches. One tablet may be cut into four pieces for flexible dosing, allowing easy titration. Directions: Dissolve tablet(s) in the mouth (on the tongue).
Ketamine Sublingual/Oral Solution
It can be used orally or sublingually, depending on the concentration and dose desired.
The advantage of using a solution is that it has a quick onset, and it is also easy for the doctor to make dosage adjustments without the patient needing a new prescription compounded. The disadvantages are that a liquid is less convenient and has a shorter expiration date than other dosage forms.
Sublingual Ketamine Troches (Lozenges)
Troches are small lozenges that can be placed and dissolved between the cheek and gum (buccally) or under the tongue to deliver medication. Troches should not be chewed or swallowed. Medication delivered this way is absorbed quicker than taking medication orally. Ketamine is naturally very bitter. Therefore, we add sweeteners and flavors to mask the bitterness. The higher the dose, the harder it is to mask the bitterness.
Rapid onset: 10-15 mins
